    Sarcoidosis: challenging diagnostic aspects of an old disease.

    Over the past few years, there have been substantial advances in our understanding of sarcoidosis immunopathogenesis. Conversely, the etiology of the disease remains obscure for a number of reasons, including heterogeneity of clinical manifestations, often overlapping with other disorders, and insensitive and nonspecific diagnostic tests. While no cause has been definitely confirmed, there is increasing evidence that one or more infectious agents may cause the disease, although the organism may no longer be viable. Here we present 2 cases, in which sarcoidosis preceded tuberculosis and non-Hodgkin lymphoma. Development of new lesions in a patient with chronic/remitting sarcoidosis should be looked at with suspicion and promptly investigated in order to rule out an alternative/concomitant diagnosis. In such cases, tissue confirmation from the most accessible site, and bone marrow biopsy-if lymphoma is in the differential diagnosis-should be performed. In conclusion, we strongly advise that physicians be ready to reconsider the diagnosis of sarcoidosis in the presence of atypical manifestations or persistent/progressive disease despite conventional therapy

    Automatic multi-trajectory planning solution for steerable catheters

    The present work describes a novel approach to trajectory planning for minimally invasive surgery consisting of an algorithm able to provide the surgeon with multiple curvilinear paths to connect an entry area defined on the brain cortex to a specific target point in the brain. A criterion based on the minimum distance from the safety-critical brain struc- tures (blood vessels, thalamus and ventricles) is used to rank the obtained trajectories. The solution is integrated onto the EDEN2020∗ programmable bevel-tip needle, a multi-segment probe whose steering ability derives from the offset generated on its tip, and provides a level of tolerance with respect to tracking errors arising from catheter model inaccuracies. The case of study of the work consists of a typical Deep Brain Stimulation scenario where tests have been performed in order to compare the result obtained from standard rectilinear trajectory planning against this novel curvilinear solution using the clearance from obstacles as an index of performance of the estimated solutions

    Appetite Regulation, Growth Performances and Fish Quality Are Modulated by Alternative Dietary Protein Ingredients in Gilthead Sea Bream (Sparus aurata) Culture

    By answering the need for increasing sustainability in aquaculture, the present study aimed to compare growth, gene expression involved in appetite regulation, physical characteristics, and chemical composition of Sparus aurata fed alternative protein sources. Fish were fed ten iso-proteic, iso-lipidic, and isoenergetic diets: a vegetable-based (CV) and a marine ingredient-rich (CF) diet were set as control diets. The others were prepared by replacing graded levels (10, 20 or 40%) of the vegetable proteins in the CV with proteins from a commercial defatted Hermetia illucens pupae meal (H), poultry by-product meal (PBM) singly (H10, H20, H40, P20, P40) or in combination (H10P30), red swamp crayfish meal (RC10) and from a blend (2:1, w:w) of Tisochrysis lutea and Tetraselmis suecica (MA10) dried biomasses. The increase in ghre gene expression observed in MA10 fed fish matched with increased feed intake and increased feed conversion ratio. Besides, the MA10 diet conferred a lighter aspect to the fish skin (p < 0.05) than the others. Overall, no detrimental effects of H, PBM, and RC meal included in the diets were observed, and fish fatty acid profile resulted as comparable among these groups and CV, thus demonstrating the possibility to introduce H, PBM, and RC in partial replacement of vegetable proteins in the diet for Sparus aurata

    Contribution of ultrasonography to the diagnosis of chronic hepatitis C histopathological changes, with emphasis on hepatic steatosis: Part I

    OBJETIVO: Avaliar a contribuição da ultrassonografia no estudo das alterações histopatológicas encontradas na hepatite crônica pelo vírus C, com ênfase para a esteatose hepática. MATERIAIS E MÉTODOS: Foram comparados os resultados dos exames ultrassonográficos do fígado de 192 pacientes portadores de hepatite crônica pelo vírus C, com os achados histopatológicos dos fragmentos obtidos por biópsia hepática. Todos os exames ultrassonográficos obedeceram a um mesmo protocolo, sendo analisados os seguintes critérios: ecogenicidade, ecotextura e atenuação. Os pacientes foram agrupados considerando-se os com alterações ultrassonográficas e os sem alterações ultrassonográficas, sendo comparados com as alterações histopatológicas presentes. RESULTADOS: Entre as alterações histopatológicas presentes, apenas os graus 0 e 3 de alteração arquitetural e a esteatose hepática apresentaram diferença estatística significante entre os dois grupos. Dentre os critérios ultrassonográficos avaliados, a atenuação foi o que apresentou melhor correlação com a esteatose hepática. CONCLUSÃO: Os resultados do trabalho demonstraram que, em pacientes com hepatite crônica pelo vírus C, a ultrassonografia apresentou limitações à caracterização das alterações histopatológicas, apresentando concordância regular com o diagnóstico de esteatose hepática. Destaca-se a capacidade do método em mostrar a probabilidade de inexistência de esteatose hepática, tendo em vista a especificidade de 77,9% e o valor preditivo negativo de 95,5%.OBJECTIVE: To evaluate the role of ultrasonography in the assessment of histopathological changes in patients with chronic hepatitis C, with emphasis on hepatic steatosis. MATERIALS AND METHODS: Liver ultrasonography results were compared with histopathological findings of liver biopsy of 192 patients with chronic hepatitis C virus infection. All the US examinations followed a single protocol, analyzing the following aspects: echogenicity, echotexture and attenuation. The patients sample was divided into two groups as follows: patients with sonographic changes and patients with no sonographic changes. Sonographic findings of both groups were compared with histopathological findings after liver biopsy. RESULTS: Statistically significant intergroup differences were observed just regarding architectural changes grades 0 and 3 and hepatic steatosis. Attenuation was the sonographic criterion that was best correlated with hepatic steatosis. CONCLUSION: The results of the present study demonstrate that, in patients with chronic hepatitis C, ultrasonography has limitations in the characterization of histopathological changes, with an intermediate rate of agreement with the diagnosis of hepatic steatosis. Considering the specificity of 77.9% and the negative predictive value of 95.5%, the authors highlight the capacity of the method to demonstrate the probability of absence of hepatic steatosis

    Automatic optimized 3D path planner for steerable catheters with heuristic search and uncertainty tolerance

    In this paper, an automatic planner for minimally invasive neurosurgery is presented. The solution can provide the surgeon with the best path to connect a user-defined entry point with a target in accordance with specific optimality criteria guaranteeing the clearance from obstacles which can be found along the insertion pathway. The method is integrated onto the EDEN2020∗ programmable bevel-tip needle, a multi-segment steerable probe intended to be used to perform drug delivery for glioblastomas treatment. A sample-based heuristic search inspired to the BIT* algorithm is used to define the optimal solution in terms of path length, followed by a smoothing phase required to meet the kinematic constraint of the catheter. To account for inaccuracies in catheter modeling, which could de- termine unexpected control errors over the insertion procedure, an uncertainty margin is defined so that to include a further level of safety for the planning algorithm. The feasibility of the proposed solution was demonstrated by testing the method in simulated neurosurgical scenarios with different degree of obstacles occupancy and against other sample-based algorithms present in literature: RRT, RRT* and an enhanced version of the RRT-Connect

    Sonographic and hemodynamic findings of schistosomiasis mansoni: doppler sonography assessment in endemic areas

    OBJETIVO: Este estudo de campo objetivou identificar as alterações ultrassonográficas e hemodinâmicas indicativas da morbidade da esquistossomose mansônica em áreas endêmicas. MATERIAIS E MÉTODOS: Foram examinados pela ultrassonografia Doppler 554 pacientes esquistossomóticos em três áreas com níveis distintos de endemicidade: baixa endemicidade (n = 109); média endemicidade (n = 255) e alta endemicidade (n = 190). Para o estudo ultrassonográfico foi utilizado o protocolo da Organização Mundial da Saúde (Niamey Working Group, 2000). Pelo Doppler foram avaliados: vasos portais, artérias hepática e esplênica, veias hepáticas e vasos colaterais. RESULTADOS: Houve correlação significativa entre a frequência das alterações ultrassonográficas e o nível de endemicidade das áreas, exceto a hipertrofia do lobo esquerdo. As veias hepáticas apresentaram padrão de fluxo alterado em 23,7% dos casos, alteração esta relacionada à presença e à intensidade de espessamento periportal. A artéria hepática não apresentou alterações nos parâmetros avaliados. Os vasos colaterais foram identificados apenas na área de alta endemicidade. A artéria esplênica apresentou alterações (aumento do calibre, da velocidade e do índice de resistência) mais frequentes na área de alta endemicidade, com diferença significativa entre os grupos. CONCLUSÃO: A ultrassonografia Doppler mostrou-se ferramenta auxiliar importante no estudo da morbidade relacionada à esquistossomose mansônica, contribuindo para definição mais precisa do perfil da doença nas áreas endêmicas.OBJECTIVE: The present field research was aimed at identifying sonographic and hemodynamic findings indicative of the presence of schistosomiasis mansoni in endemic areas. MATERIALS AND METHODS: Doppler sonography was performed in 554 patients with schistosomiasis in three areas with different endemicity levels: low (n = 109), medium (n = 255) and high endemicity (n = 190). The World Health Organization (Niamey Working Group, 2000) protocol was adopted for sonographic evaluation. Doppler study included portal vessels, hepatic and splenic arteries, hepatic veins and collateral vessels. RESULTS: A significant correlation was observed between the frequency of sonographic findings, except for left lobe hypertrophy, and the areas endemicity levels. Altered hepatic veins flow pattern was observed in 23.7% of cases, such abnormality being related to the presence and intensity of periportal thickening. Hepatic arteries did not present any alteration as related to the evaluated parameters. Collateral vessels were identified only in the patients from the high-endemicity area. The splenic artery presented alterations (increase in caliber, flow velocity and resistive index), most frequently in the high-endemicity area, with significant difference between groups. CONCLUSION: Doppler sonography has shown to be a relevant auxiliary tool in the study of the morbidity related to schistosomiasis mansoni, contributing for a more accurate description of the disease profile in endemic areas

    Deep soils modify environmental consequences of increased nitrogen fertilizer use in intensifying Amazon agriculture

    Agricultural intensification offers potential to grow more food while reducing the conversion of native ecosystems to croplands. However, intensification also risks environmental degradation through emissions of the greenhouse gas nitrous oxide (N2O) and nitrate leaching to ground and surface waters. Intensively-managed croplands and nitrogen (N) fertilizer use are expanding rapidly in tropical regions. We quantified fertilizer responses of maize yield, N2O emissions, and N leaching in an Amazon soybean-maize double-cropping system on deep, highly-weathered soils in Mato Grosso, Brazil. Application of N fertilizer above 80 kg N ha−1 yr−1 increased maize yield and N2O emissions only slightly. Unlike experiences in temperate regions, leached nitrate accumulated in deep soils with increased fertilizer and conversion to cropping at N fertilization rates \u3e80 kg N ha−1, which exceeded maize demand. This raises new questions about the capacity of tropical agricultural soils to store nitrogen, which may determine when and how much nitrogen impacts surface waters

    Cyclocreatine treatment ameliorates the cognitive, autistic and epileptic phenotype in a mouse model of Creatine Transporter Deficiency

    Creatine Transporter Deficiency (CTD) is an inborn error of metabolism presenting with intellectual disability, behavioral disturbances and epilepsy. There is currently no cure for this disorder. Here, we employed novel biomarkers for monitoring brain function, together with well-established behavioral readouts for CTD mice, to longitudinally study the therapeutic efficacy of cyclocreatine (cCr) at the preclinical level. Our results show that cCr treatment is able to partially correct hemodynamic responses and EEG abnormalities, improve cognitive deficits, revert autistic-like behaviors and protect against seizures. This study provides encouraging data to support the potential therapeutic benefit of cyclocreatine or other chemically modified lipophilic analogs of Cr
